Exam Script (TECA) #

ExamCA Handoff

Hello, PATIENT NAME, my name is CA NAME, and I’m excited to help you with your exam today. Thank you for filling out our update form. Please follow me back, I have everything ready for you.

In the Room

This is an important check-in at this phase of your care because it helps us to know what areas we need to focus on to help you get the best results and reach your goals.

💡
Make it personal. Mention a life effect or symptom from their update paperwork.
Rolling Thermal Scan

Ok, PATIENT NAME, to get started we are going to do a “Rolling Thermal” scan. This is going to measure your nervous system and tell us how well your brain is able to send signals to every other system in your body. We are looking for as clear a scan as possible with little to no pattern from your last scan. Please sit with your feet on the floor. Facing forward with your knees shoulder width apart and your hands placed open, palms up on your knees. This will only take a few seconds.

EMG

This next test will measure the tension in your muscles. It will help us know if you have an imbalance in the muscles along the spine. This is one of the results the doctor uses to make a decision on the frequency of your chiropractic care as well what exercises to recommend to you.

For this one, please sit as you did, feet on the floor, facing forward with your hands palm up on your knees. Be as still and quiet as you can for this one so we get the most accurate results possible.

Heart Rate Variability

Our next exam is going to measure how your body is adapting to stress. Just place the three middle fingers of into this device here and sit as still and quiet as you can. When I click start the computer will asses the signals it needs from your fingers and then I’ll click start again. At that point it will count down for ten seconds and then begin the 3 minute test. Again, sit as still and quiet as you can for this test so that you results will be as accurate as possible.

No X-Rays/In-Body

When the test is completed, a window will pop up on the computer screen. You don’t have to do anything with that, just remove your hand, get dressed (if not already) and come on out to the black chairs in the adjustment area. We will have you signed in and the doctor will be watching for you to get you adjusted.

X-Rays/In-Body

When the test is completed, a window will pop up on the computer screen. You don’t have to do anything with that, just remove your hand, get dressed (if not already) and come on out to the black chairs in the adjustment area. I’ll take you back from there to get some updated x-rays for the doctor to compare to your previous ones. This will give him information on your structural progress.

X-Rays (Re-Exam Only)

We are going to take the same x-rays we did at the beginning of your care so we can see how you’re progressing from then to now.

🛑
Ask patient to remove their shoes, belt, and any metal jewelry.
Lat Cervical

For this first x-ray we are going to take a side view of your neck. Come stand close enough to the board that you can feel the board with your arm. I will line you up. For this x-ray you can just look straight at the wall and hold really still for me.

AP Cervical

For the front view of you neck I will have you turn and face me with your back against the board, close enough where you can feel it. Close your eyes so this light doesn’t shine in them. I’m going to make sure everything is lined up. When I get ready to take the picture I’ll have you open your mouth nice and wide so we can see the vertebra behind your teeth.

Lateral Thoracic/Lumbar

We’re going to take a side view of your thoracic/lumbar spine. I’ll have you turn and face the wall like before and for this x-ray, please hold your arms up like this (demonstrate) so that I can line everything up. Now I’ll take a quick measurement and when I am ready to take this picture I’ll let you know and I’ll have you hold your arms up, take a deep breath in, blow it all out and hold. I’ll let you know when I’m ready for you to do that. You can relax your arms while I get everything set up but please don’t move.

AP Thoracic/Lumbar

We’re going to take a front view of your thoracic/lumbar spine. I’ll have you turn and face me and make sure you can feel the board behind you. Now I’ll take a quick measurement and when I am ready to take this picture I’ll let you know and I’ll have you take a deep breath in, blow it all out and hold. I’ll tell you when I’m ready for you to do that. You can relax your arms by your side for this one. Please hold as still as you can.

The doctor will go over these x-rays with you at your report just like they did at the beginning.
