New Patient Exam: Patient receives exam and consultation to determine what phase of care is necessary to achieve their personal health goals.

Report of Findings: D.C. explains the results of scans and x-rays and presents their recommended plan designed by the D.C. to achieve the patient’s health and wellness goals.

Activation of First Plan Patient agrees to follow D.C. recommendations, signs the agreement, schedules appointments out, and pays for their initial plan of care either in one full payment or by signing up for monthly payments.

Progress Report (Halfway through “Calendar” length of plan.) D.C. explains the results of the follow-up scan and updates the patient on their progress. Increased care or nutrition and fitness recommendations may be included in the Progress Report.

Re-Report (New Game Plan [R1]) (End of care plan completed visits.) D.C. discusses the results of the re-exam and x-rays with the patient and provides updated recommendations of care based on the evidence in the new exam. This plan will be entered as R1 regardless of recommendations.

R1: Renewal #

Activation of Corrective Plan The patient agrees to follow D.C. recommendations, signs the agreement, schedules out appointments, and pays for their Corrective Care plan either by paying in one full payment or signing up on monthly payments.

R4: Wellness Club #

Activation of Wellness Plan Patient agrees to follow D.C. recommendations, signs the agreement, schedules out appointments, and pays for ongoing plan of care by signing up for annual or monthly payments. Payments are set up by BOCA to be withdrawn regularly according to the patient agreement. [The “Wellness Club” charge should be put in their ledger and their Case Type should be changed.]

Progress Reports D.C. explains results of follow-up scan and updates patient on their progress. A “report of x-rays” will be scheduled with the Progress Report if comparative x-rays were requested by the D.C. Increased care or nutrition and fitness recommendations may be included in the Progress Report. Progress Reports take place every six months on a wellness plan. Financial discussions will also be necessary bi-annually but will only include payment for extra care and updated permission to charge the annual or monthly fee for the Wellness Care Plan. (These financials can often be completed at the front desk.)

Re-Report (New Game Plan) by D.C. Request, Accident/Injury, or Special Circumstance Only D.C. discusses results of re-exam and x-rays with the patient and provides updated recommendations of care based on the evidence in the new exam. This new “Game Plan” may be a transition to a temporary Intensive or Corrective plan based on evidence found in scans and x-rays. A financial conversation will be necessary for this report.
