New Patient: #

❗The Office Policies will be presented at the BRF and signatures will be obtained then. The patient will get their sign in number on Day 3 and their office policy sheet will be made and attached to the BRF clipboard under the appropriate sign up list.

Scripting for Office Policies should follow the pattern below whether in front of the BRF or in person with a patient: #

PATIENT NAME, the doctors’ recommendations are based on your goals and his findings. This means that you will get the most out of your care plan if you follow his recommendations.

To help you stay on track we have these office policies simply listed for you here on one page. Look over it and we’ll briefly go over how we help you stay on track with doctor recommendations.

Show patient the HFC_Office_Policies_2025 document and allow them time to read over it.

First of all, your recommendations are designed for you to receive the number of adjustments recommended before the end date of your plan which is written here. We will do everything we can to help you keep to these recommendations. For example, if you miss an appointment we’ll reach out to you immediately to try and reschedule. If we can’t get ahold of you we will move that visit to a similar time on a different day within the same week. You can always let us know through text or a call if that appointment needs to be rescheduled.

We also have a three strike rule in our office. This means that if you miss three appointments in a row without contacting us to reschedule, we will freeze your account and move forward with cancellation. We will try to reach you again and send out a cancellation letter outlining any of your ledger details.

If at any point you feel like you need extra care. Maybe a off schedule adjustment or a laser or decompression is recommended by the doctor. That care will be charged on the day of your extra visit but your adjustment will be at your care plan discount price. Therapies and massage are at regular price.

Your doctor will have mentioned our Better Results Faster Class. This is a great benefit to our patients and the next important step in your care.
